Refer
Hospital discharge planners, physicians, case managers, or family members can initiate a referral by calling or faxing patient information.
Assess
Our clinical team reviews medical records, current condition, and care needs to confirm we're the right fit and determine the appropriate level of care.
Admit
We coordinate transportation, room preparation, and admission paperwork — and meet the patient and family on arrival to ensure a calm transition.
Care
Our interdisciplinary care team begins delivering the personalized care plan, with family kept informed every step of the way.
